Health insurance is not something to skip as an expat in Thailand. While day-to-day healthcare is affordable, serious illness or a major accident can generate hospital bills of hundreds of thousands of baht. A good international health insurance policy protects you against that.
Do You Need It?
Practically speaking, yes. The Thai government previously required proof of health insurance for the retirement visa, but this requirement was relaxed. However, the financial case for having insurance remains strong. A week in a Thai private hospital for something serious can cost 200,000 to 500,000+ THB. Insurance makes that manageable.
What to Look For
- Inpatient cover: Essential. Covers hospitalisation, surgery, and emergency treatment.
- Outpatient cover: Covers GP visits, consultations, and diagnostics. Useful but not always cost-effective depending on your usage.
- Dental and vision: Often sold as add-ons. Compare costs against paying out of pocket, dental in Thailand is so cheap that insurance may not be worth it.
- Emergency evacuation: Important if you want the option to return home for treatment.
- Pre-existing conditions: Declare accurately. Undisclosed conditions can invalidate claims.
- Thailand-only vs international cover: Thailand-only plans are cheaper. International plans cover you when you travel.

Rough annual premiums for expats in Thailand (2026 estimates):
- Age 40 to 50: 40,000 to 70,000 THB/year for a solid inpatient-only plan
- Age 50 to 60: 60,000 to 100,000 THB/year
- Age 60 to 70: 90,000 to 150,000 THB/year
Premiums increase annually. Buy earlier rather than later when rates and underwriting terms are more favourable.
